About the policy
Faculty and academic staff may authorize someone to check out library materials in their name. Students with a physical disability who are registered through the Disabled Students Program may also authorize someone to check out library materials in their name.
Under this arrangement, the individual faculty, academic staff member, or UC Berkeley student with a physical disability agrees to be responsible for responding to recall notices, returning/renewing materials on time, and paying any replacement bills for books that are lost or damaged.
To apply for or renew a proxy card:
- Faculty or academic staff member must submit a faculty/staff application form.
- Students must submit a student application form.
