How to Submit Duplication Requests

  • Duplication Request Form: Request duplications from monographs, journals, and other formats. Also use this form when requesting digital reproductions of microfilms.
  • Map Duplication Request Form: Request duplications of maps.
  • OCLC Requests: Accepted from other institutions, organizations and libraries. Please list CUY twice consecutively in the Lender String.
  • Fax requests (preferably typed): Use an ALA form, company letterhead, or purchase order and fax to (510) 643-8476.
  • Mail-in requests (preferably typed): Use the standard ILL form (preferred), official letterhead or purchase orders. Please submit a separate request for each journal or monographic title with your complete address on each page.
  • Phone requests: not accepted.
