Hello all,
I am resending the following message now that the survey is back up and
running. If you tried to submit the survey previously (and great thanks to
those of you who did) and received an error message when you submitted it I
do not have your information. The survey is now on a completely different
web server, and appears to be stable, so I've changed the URL in the
message below to reflect the new location...
Thanks very much for your patience and help,
Mary

Dear Colleagues,

Barb Bergman, Media Services Librarian at Minnesota State
University-Mankato, and I are working on a collaborative project
researching current collection and circulation policies for academic media
centers. Your assistance with a brief survey would be greatly appreciated.

The online survey will only take 5-10 minutes of your time, and is
completely voluntary. You may discontinue participation at any time. Feel
free to contact me or Barb with any questions regarding the survey, or you
can contact the UIUC Institutional Review Board Office (irb@uiuc.edu) with
questions about participants' rights.

All information gathered is strictly confidential and anonymous. Data
gathered from the survey will be summarized and hopefully published in
professional papers and/or presented at professional meetings.

To access the survey, please go to
<http://web.library.uiuc.edu/ugl/media/survey/policysurvey.htm>. Please
note that you must be 18 or older to participate in this study. Thank you
for taking the time to help with this research project, we realize this is
a busy time of year.
