[Videolib] bizarre form of the day

As some of you may recall I was upset last year and refused to sign
a form from an institution in California requiring me to "certify" that
Kino was a "drug free" office. I thought it was both silly and insulting
I just got one that is much stranger though I don't have any objection to
signing it. A small nameless public library requires that I sign a form
basically certifying I don't get kickbacks on any orders and that the order
is legit. The wording is especially awkward and might require someone who
receives commissions on sales to either lie or not sign the form.

I just it silly but it is an extra bit of wasted time & paperwork
Anyone else got forms like this?
