Instructions for UC Berkeley Library Card Applications and Renewals

This service is provided to OEHHA personnel only.

OEHHA personnel may request UC Berkeley library cards. A library card allows you to check out books from the UC Berkeley Library, subject the Library's circulation policies. There are separate instructions for new cards and renewals.

In order to process your requests for books, we must enter your name and home address into the circulation database. New cards and renewal stickers require a signature from your branch chief or unit head.

If you are an employee who is leaving OEHHA, please mail your library card back to us so that we can properly dispose of it. Please also include your name along with the library card you are returning to us.

Instructions for New Library Cards:

If you have never had a UCB library card, please print and fill out the library card application form. Library cards that have been issued are permanent and barcoded with a unique user identification number. If your card is lost or stolen, the University will charge you a $10 replacement fee. Please report all lost or stolen cards immediately to Judy Bolstad at (510) 642-2510 or via e-mail jbolstad@library.berkeley.edu.

Library cards will be mailed directly to Branch or Division Chiefs for distribution to personnel.

You must print out and complete the library card application form.

Instructions for Library Card Renewals:

  1. Please submit a list of staff members who were issued library cards in previous years on the enclosed library application form. Please check this list to ensure that it correctly reflects current employment. Library cards will automatically be renewed when the list is returned to the Public Health Library, and you will then receive stickers updating the cards. Make a copy of this list for your records so that you know who should receive renewal stickers.
  2. New appointees to the Department or personnel who did not receive library cards previously should have their names typed in alphabetical order on the application form. Indicate "NEW" by their names. Branch chiefs or unit heads should sign the application forms and return them to the Public Health Library. Copies of the blank application form should be made and retained in each section or unit for use by new employees hired during the year.

Renewal stickers will be mailed directly to Branch or Division Chiefs for distribution to personnel. If you would like library cards for UC campuses other than Berkeley, contact your Branch or Division Chief.
